What the evidence says 01 / RECORD

The approximately 40-41% figure widely cited for transgender suicide comes from the 2008-2009 National Transgender Discrimination Survey and its 2015 successor (the U.S. Transgender Survey), which asked respondents whether they had ever attempted suicide in their lifetime, not how many transgender people die by suicide. That survey relied on a self-selected, non-random sample with no matched non-transgender comparison group, a limitation noted by researchers who use the data. Population-level mortality data on completed suicide by transgender status do not exist because death certificates do not record gender identity, so no verified death rate near 40% can be confirmed. Contrary to the claim that surgical complications drive suicide risk, a secondary analysis of the 2015 U.S. Transgender Survey (n=27,715) found that transgender adults with a history of gender-affirming surgery had significantly lower odds of past-year suicidal ideation than those without such surgery, not higher. Current evidence supports an elevated rate of self-reported lifetime suicide attempts among transgender survey respondents, but the specific 40% figure is frequently misapplied as a suicide death rate, and the surgery-complications explanation is not supported by available data.
